Revision Surgery for Face and Neck lift with Pelican Surgery done in Korea

Today, we performed a revision face and neck lift, including a platysmaplasty or pelican surgery at our clinic for a patient who undergone the same procedures in Korea.

The procedures performed were the following
- Incisional Lift
Deep Plane Facelift
Dual Plane Mid Facelift
Lateral Neck Lift
Pelican Surgery (Platysmaplasty)
Regarding the Pelican Surgery, the subcutaneous fat above and below the platysma muscle had already been appropriately removed by the previous surgeon.
Our clinic's strategy was to further reduce the volume and re-tighten of the submental area.
When the mandible is small, the submental tissues will sag due to the volume overload.
Therefore we reduced the volume of the mylohyoid muscle, the posterior belly of the digastric muscle, and the platysma muscle.
Additionally, the digastric muscle was newly tightened in a corset-like procedure from the musculotendinous junction and the platysma was sutured to the periosteum of the hyoid bone to create an angle.
The facial contour - was lifted with Lateral Neck Lift and Deep Plane Facelift.
As for the neck lift, compared to preservation facelift and extended deep plane facelift, our neck lift technique elevates the central part of the neck, to prevent the sagging of the submandibular gland.
If the submandibular gland is palpable before the surgery, a partial resection of the gland is performed on the outside or submental area,
In addition to the previous facelift, lifting of the buccal fat was also performed to elevate the tissues beneath the SMAS layer.
If the buccal fat is large, partial resection is performed here too, then lifted up prior to suspension. In this case, the volume was reduced and lifting was performed on the left side.
For the Midface - our approach focuses on lifting up the fat compartments.
In Japan, we occasionally see Deep Plane Facelift cases where the patient is not satisfied with the elevation of the midface.
We believe the midface in Asian patients is considered an area particularly susceptible to the effects of gravity.
Repositioning the fat components has become a growing trend globally.
